Navigating Elder Care Insurance and Medicare Coverage for Home Care Services

Most elderly people prefer home care services because they can receive personalized support in the comfort of their homes.

However, this convenience comes at an extra cost. You may need to invest in medical supplies, mobility aids or hire skilled caregivers. Moreover, some insurance plans, including elder care insurance, provide limited coverage for home care, which means you may have to cover many expenses out of pocket.

One option that can help ease this financial burden is Medicare. This is a federal health insurance program for individuals aged 65 and older and those with certain disabilities.

Eligibility Requirements for Medicare Home Care Services

You must have an initial in-person visit with your doctor or a qualified healthcare provider. This visit must occur 90 days before starting home care or 30 days after starting the care.

The doctor should provide a report that confirms that you are homebound. The report will also state that you require skilled care, outline the services you need, and explain how they will be provided.

You must also receive the services from a Medicare-certified home health agency. Medicare sets strict guidelines for these agencies to ensure they meet quality standards. The home health agency will conduct an initial assessment at your home to further evaluate your condition and develop a personalized care plan. They will coordinate with your doctor to offer services that align with your doctor’s instructions.

Moreover, the services must be considered medically necessary. This means they must be essential to treat or manage a specific health condition. Examples include skilled nursing care for wound care, injections, or IV therapy. It won’t cover services like cooking, cleaning, or maintaining the care homes. You may need to cover these services out of pocket.

Lastly, your care must be part-time or intermittent. It cannot be around the clock. This ensures that your elder care insurance covers only essential medical care rather than full-time caregiving.

What Home Care Services Does Medicare Cover?

Medicare offers different health care covers, which are categorized from Part A to Part D. Their home care services are usually covered under Part A and Part B. Part A generally covers inpatient hospital care and also provides limited coverage for home health services under certain conditions.

On the other hand, Medicare Part B covers outpatient care and home health services for those who qualify. You can only receive home health services if your doctor certifies that you need them and are considered homebound. Unlike Part A, which is tied to hospital discharge, Part B does not require a recent inpatient stay.

Here are some of the home care services offered by Medicare:

Skilled Nursing Care

This includes part-time services from a registered nurse or licensed practical nurse for medical conditions like wound care, injections, or IV therapy. You will only receive about eight hours per day and less than 28 hours per week, with a temporary extension of up to 35 hours in special cases.

Therapy Services

Medicare covers physical, occupational, and speech therapy if they are part of a doctor-approved care plan and are considered medically necessary. Physical therapy improves movement and balance, while occupational therapy can help you perform daily activities such as cleaning, dressing, and eating. Speech therapy addresses communication difficulties or swallowing disorders.

Medical Social Services

A social worker will help you understand your Medicare benefits, explore additional support options, and cope with the emotional stress of a medical condition. This ensures you receive medical care and the guidance and support needed to maintain a good quality of life at home.

Home Health Aide Services

These services offer personal care support for individuals who need help with daily activities due to aging, illness, injury, or disability. They may include assistance with bathing, dressing, grooming, and mobility to ensure you maintain your comfort and hygiene while recovering at home. However, Medicare only covers home health aide services if they are part of a doctor-approved care plan and are provided alongside skilled medical care, such as nursing or therapy. As such, you cannot receive home health aide services alone without a medical need.

Durable Medical Equipment (DME)

Medicare can help cover the cost of medically necessary equipment like walkers, wheelchairs, hospital beds, or oxygen supplies. These devices improve mobility, safety, and comfort, making it easy to deliver quality home care services.

Telehealth Services

This can include virtual visits with doctors, nurses, or therapists who can monitor your condition, provide medical advice, or adjust your care plan without needing in-person visits.

The Duration of Medicare Home Care Coverage

Medicare does not set a strict time limit on how long you can receive home care services. Instead, the duration of coverage is determined by your ongoing medical needs and the periodic reassessment of your condition.

Initially, your doctor must certify that you need home health care, and this certification is valid for 60 days. At the end of this period, your home health agency must reassess your condition and submit a new certification if you still qualify for services. This process can continue once your doctor confirms that you require skilled care and meet Medicare’s criteria.

However, Medicare coverage may end if your condition improves to the point where skilled care is no longer necessary. On the other hand, if your condition stabilizes but you still need skilled care to prevent it from worsening, Medicare can continue covering your home care.
